The Publication Scheme is required by the Freedom of Information Act 2000

The Publication Scheme is a guide to the information routinely made available to the public by Powell & Associates Dental Practice. It is a description of the information about our dental practice which is made available to the public. Some information is withheld, but we review this publication at regular intervals and will monitor any changes. A full list of publication are available on our practice website or by contacting the Practice Manager – Agnes Powell or Julie Wroe

How much does it cost?
All publications are free unless otherwise stated

Your access to information

As well as our published information, past and present patients of the practice have the right to access their own personal information, in accordance with the Data Protection Act 1998. Details of how to do this is contained in our Data Protection Policy and this is made available by our Practice Manager Agnes Powell / Julie Wroe

Classes of Information

We hold various types of information that we review, retain or dispose of in accordance with NHS rules. This information is classed into seven categories:

Class 1: Who we are

Details of all the dentists, dental hygiene – therapists are in the practice information leaflet. Details include name and date of registration with the General Dental Council, any specialist status and days that they work for the practice. The name of the practice manager is also included

Class 2: Our Services

Information about our services is held in the Practice’s patient leaflet that is available in Reception & can be viewed on the practice website

  • Opening Times
  • Arrangements for emergency care
  • Details of access to the premises for people with disabilities
  • Dental, Hygiene & orthodontic treatment available
  • Information about extraction after care, orthodontic advice and dietary advice sheets

Patients are offered all standard NHS treatment, and private treatment if required

Class 3: Financial Information

We have information about NHS Charges

  • Entitlement to exemptions and remission from NHS DENTAL CHARGES
  • Children under 18 & young people in full time education who are under 19
  • Pregnant women, women who have had a baby in the last 12 months
  • Income Support
  • Income Based Jobseeker’s Allowance
  • Income – related Employment & Support Allowance
  • Tax Credit
  • Pension Credit Guaranteed Credit
  • HC2 or HC3 certificates
  • You can apply for help with NHS dental costs by filling out an HC1 Form
  • Private charges are discussed with each patient prior to treatment being undertaken
  • Our NHS funding is an annual budget based on historical earnings. Patients who are not exempt must pay for their dental treatment, payment is due on the day treatment is undertaken

Class 5: Complaints

We have a practice complaints procedure, this is on view on the Patients notice board in the waiting room, a hard copy is available from Agnes Powell and Julie Wroe
